My Philosophy of Education As I start to write this paper, I begin to think of the reasons why I want to become a teacher. The most important reason is my love for children. When I was little, I always dreamed about being a teacher and I would always pretend to be teaching a classroom. As I grew older, I knew I wanted to be in a field that involved interaction with people. I soon began to work at a local gymnastics center where I became a coach for children form ages two to seventeen. At my work, even through the bad experiences, I could not help but love and want to help those kids even more. This was a great experience because I discovered how much I wanted to be a part of helping children to learn and succeed. I plan to use many different ideas in the way the class is arranged. The bulletin boards will be very colorful and one may include studen t info, like their pictures, names, birth dates, and interests while the other may be seasonal. I believe that an effective teacher will use both authoritarian and non - authoritarian instruction, depending on the content and the situation. I would want the kids involved but when discipline arises I would become strict. I plan to be well organized and I plan to present the subject matter in an interesting manner. I will also try to bring in a lot of variety because it will increase a student’s curiosity. On the first day of school, I plan to let students know what is expected of them and what they can and cannot due. To motivate my students I will learn their names quickely and provide encouragement, as well as, positive reinforcement. In dealing with discipline, I plan to set down with my students on the first day of school and develop a list of rules and consequences. I plan to enforce these rules with everyone; if I would make an exception, I would be unfair. As The World Grows So Do Our Challenges Essay As the world grows so do our challenges. Not only are our problems growing in number but they are also growing in complexity. There is scarcely a day that goes by when I dont hear about some new problem that will soon threaten us all. While some of these problems such as El Nino, earthquakes, and other natural disasters cannot be stopped there are other challenges such as the spread of racial hatred, rising risk of disease spread, and the growing levels of civil unrest throughout much of the world which threaten many lives everyday. Most of the futures challenges will only be made possible if all people, regardless of race, color, or nationality, attempt to work together for a stronger world. There are many ways in which I believe that I can help society meet this future challenge of coming together as a united world. While I was an exchange student, I saw the world from a different point of view. For the first time, I realized that the people of the world have so many similarities in comparison to the few and insignificant differences. To make the people of the world unite would merely take time in convincing people of the world that we are all in pursuit of the same basic goals. All that it would take to show that this could be done is two different cultures to unite and then the rest of the world what can come by the strength all gain by coming together. I believe that I can help in solving some of the barriers that hold us back from completing the task that we all desire which is unity. I served as a miniature version of a diplomat when I was an exchange student. I helped people in another country see that we all have common goals and I also helped sever the bonds to racial hatred by providing them insight into the American way of thinking and they did likewise for me. I have done everything to help sever these ancient bonds and will continue to do so in the future as I feel I am called to do. Many suffer problems on a much less grandiose scale. Although these problems may not seem to be as big as world peace or global warming, they are however present. People throughout the world suffer from missing limbs, nervous disorders and a myriad of other health problems. I have always dreamed of helping those who cannot help themselves. This desire is part of what has inspired me to become a Biomedical Engineer for I wish to help design medical devices that are practical and useful for the disabled. Through the advancement of technology, I hope to give the disabled a greater sense of self-worth and better equip them to fit into society. I believe that doing so would be incredibly rewarding, not only to myself, but also, to those whom I have the opportunity to touch. I believe that my diversified  background and also my desire to develop technology for the advancement of others will help to make me useful in solving some of the problems that the future will entail.

It is easiest to understand how electron transport works by dividing this process into three main events: 1.Oxidation Reduction Reactions During glycolysis, synthesis of acetyl-CoA and Kreb's cycle, the electron carriers NAD+ and FADH are reduced to form NADH and FADH2 respectively. These molecules are like little rechargeable batteries, and when NAD+ and FADH are reduced, this means that they accept and carry electrons and hydrogen ions (H+), potential energy that can be used later in cellular respiration. In the electron transport chain, these electron carriers are oxidized, transferring their electrons to the carrier molecules embedded in the ETC membrane. In aerobic respiration, these electrons are passed from one carrier molecule to another in a series of oxidation-reduction reactions, and ultimately to the final electron acceptor, oxygen (O2), that combines with hydrogen, resulting a water (H2O), a metabolic waste product. 2.Creation of Hydrogen Ion Gradient The energy from each electron being passed down the chain is used to pump a proton (H+) through each carrier molecule, from one side of the membrane to the other. This creates a proton gradient, a type ofconcentration gradient(difference in concentration of a substance between two sides of a membrane), and gradients are potential energy available for cellular work. 3.Phosphorylation of ADP(The payoff!) The hydrogen ions (H+), on the side of the membrane where most concentrated, will eventually flow back across the membrane, down the proton gradient, through anenzymecalledATP synthase. Do You Qualify for the National Merit Scholarship SAT / ACT Prep Online Guides and Tips Out of the 1.5 million juniors in the US who take the PSAT only about 7,600 will end up with a National Merit Scholarship of $2,500 through the National Merit Scholarship Program. So how do you qualify? If you do qualify, what are your chances of getting the scholarship? I am going to answer these questions by discussing the basic qualification for National Merit and laying out a timeline for the program. Qualification Guidelines There are four main qualifications for scholarship consideration: three basic and one PSAT score related. They are all equally important because if you do not meet all of the qualifications, you cannot participate in the program.The 3 basic qualifications you need to meet are: Take the PSAT during your junior year (3rd year) of high school Plan to enroll in college full-time by the fall after your graduate from high school Be a US Citizen or US Permanent resident who plans to become a US Citizen Then there is the test score related qualification. The exact score qualifications vary from year to year and state to state, but typically, the top 3% of students (about 50,000 juniors) qualify to be in the program. The top 1% of students have the best shot at winning the scholarship. How Does the Selection Timeline Work? Let's go through when each step of the National Merit selection occurs and how it will affect you if you're chosen. October of Junior Year: You take the PSAT. April of Junior Year: 50,000 students are notified that they are potential National Merit Scholars and are asked to select 2 colleges thatthe National Merit Scholarship Corporation (NMSC) will send a reference. September of Senior Year: â…” of the 50,000 students (about 34,000) will receive Letters of Commendation (recognizing them for their academic achievement, but notifying them they are no longer in the running for a scholarship). The remaining â…“ of students (about 16,000) will advance as National Merit Scholarship Semifinalists. Semifinalists typically have PSAT scores of at least 201 to 222 (on the old 240 point scale). Again the reason for the large point discrepancy is that the winners are chosen by state. If your state has lower scores on average, you will be able to qualify with a lower score than in other states with higheraverage PSAT scores. The Semifinalists are asked to submit applications for scholarship considerations. I will not go into detail on the scholarship application in this article. For more information on what is required for the National Merit Scholarship application and how to craft the strongest application read our other article. February of Senior Year: Semifinalists who advance to the Finalist round will be notified. About 15,000 of 16,000 Semifinalists advance. March of Senior Year: The 8,900 Finalists who are selected for a scholarship (out of the 15,000 Finalists) will be notified. NOTE: not all of these 8,900 Finalists receive a $2,500 one-time National Merit Scholarship. Through the program, students may be offered a National Merit Scholarship, a Corporate-Sponsored Merit Scholarship, or a College-Sponsored Merit Scholarship. (I will explain how students are selected for scholarships in more depth below.) How Do You Know If You Probably Won’t Get a Scholarship? Obviously, if you are not notified in April or if you receive a Letter of Commendation in September, you know you will not be advancing to the Semifinalist round. Therefore, youwill not be in contention for a National Merit Scholarship or one of the other scholarships. Additionally, if your score is not in the top 1%, you don't have a great chance of advancing to the Semifinalist round (the â…” of students who receive Letters of Commendation and do not advance to Semifinalists typically have scores in the top 2-3%).Once you have advanced to the Semifinalist round, you have about a 50-50 chance of receiving a scholarship (since half the Semifinalists receive scholarships: 8,900 of 16,000). How Do You Know If You Have a Good Chance of Getting the Scholarship? If your score is in the top 1% (for your state), you have a good chance of getting the scholarship.However, the final step in consideration for the scholarship is your application and the National Merit Scholarship Program acknowledges that scholarship recipients are chosen â€Å"based on their abilities, skills, and accomplishments.† Once you are selected as a Semifinalist and submit your application, the National Merit Scholarship Corporation takes all of the following into consideration when deciding on scholarships:â€Å"the Finalist's academic record, information about the school's curricula and grading system, two sets of test scores [SAT and PSAT], the high school official's written recommendation, information about the student's activities and leadership, and the Finalist's own essay.† If you have a very strong transcript (as close to 4.0 GPA on 4.0 scale with challenging course load of AP or IB classes) and a very high SAT score (close to 2400 on the old scale or close to 1600 on the new SAT),in addition to having a PSAT score in the top 1%, you have a solid chance of receiving a scholarship. According to the National Merit website, National Merit Scholarships are $2500 single payment scholarships, which are awarded â€Å"without consideration of family financial circumstances, college choice, or major and career plans.†All Finalists are considered for this scholarship. Corporate-sponsored Merit Scholarship awards are awarded based on each Corporate sponsor’s criteria.Some â€Å"designate their awards for children of their employees or members, for residents of a community where a company has operations, or for Finalists with career plans the sponsor wishes to encourage. These scholarships may either be renewable for four years of undergraduate study or one-time awards.† The amount for these Corporate-sponsored Merit Scholarships varies by company. College-sponsored Merit Scholarship awards are awarded to â€Å"Finalists who have been accepted for admission and have informed NMSC by [through their application] that the sponsor college or university is their first choice.†You can only receive one of these College-sponsored Merit Scholarships if you were accepted to the university that you told National Merit was your first choice. Also, if you were accepted to your first choice college and choose not to attend that university, you will forfeit your scholarship. The amount of these scholarships vary by school, but each scholarship will be between $500 and $2000 per year. These scholarships are renewable up to 4 years. You have little control over which scholarship you will receive. All three of types of scholarships are equally prestigious and receiving one is no better than receiving another from an academic standpoint (though it may be better from a financial standpoint). Chemosynthesis Definition and Examples Chemosynthesis is the conversion of carbon compounds and other molecules into organic compounds. In this biochemical reaction, methane or an inorganic compound, such as hydrogen sulfide or hydrogen gas, is oxidized to act as the energy source. In contrast, the energy source for photosynthesis (the set of reactions through which carbon dioxide and water are converted into glucose and oxygen) uses energy from sunlight to power the process. The idea that microorganisms could live on inorganic compounds was proposed by Sergei Nikolaevich Vinogradnsii (Winogradsky) in 1890, based on research conducted on bacteria which appeared to live from nitrogen, iron, or sulfur. The hypothesis was validated in 1977 when the deep sea submersible Alvin observed tube worms and other life surrounding hydrothermal vents at the Galapagos Rift. Harvard student Colleen Cavanaugh proposed and later confirmed the tube worms survived because of their relationship with chemosynthetic bacteria. The official discovery of chemosynthesis is credited to Cavanaugh. Organisms that obtain energy by oxidation of electron donors are called chemotrophs. If the molecules are organic, the organisms are called chemoorganotrophs. If the molecules are inorganic, the organisms are terms chemolithotrophs. In contrast, organisms that use solar energy are called phototrophs. Chemoautotrophs and Chemoheterotrophs Chemoautotrophs obtain their energy from chemical reactions and synthesize organic compounds from carbon dioxide. The energy source for chemosynthesis may be elemental sulfur, hydrogen sulfide, molecular hydrogen, ammonia, manganese,  or iron. Examples of chemoautotrophs include bacteria and methanogenic archaea living in deep sea vents. The word chemosynthesis was originally coined by Wilhelm Pfeffer in 1897 to describe energy production by oxidation of inorganic molecules by autotrophs (chemolithoautotrophy). Under the modern definition, chemosynthesis also describes energy production via chemoorganoautotrophy. Chemoheterotrophs cannot fix carbon to form organic compounds. Instead, they can use inorganic energy sources, such as sulfur (chemolithoheterotrophs) or organic energy sources, such as proteins, carbohydrates, and lipids (chemoorganoheterotrophs). Where Does Chemosynthesis Occur? Chemosynthesis has been detected in hydrothermal vents, isolated caves, methane clathrates, whale falls, and cold seeps. It has been hypothesized the process may permit life below the surface of Mars and Jupiters moon Europa. as well as other places in the solar system. Chemosynthesis can occur in the presences of oxygen, but it is not required. Example of Chemosynthesis In addition to bacterial and archaea,  some larger organisms rely on chemosynthesis. A good example is the giant tube worm which is found in great numbers  surrounding deep hydrothermal vents. Each worm houses chemosynthetic bacteria in an organ called a trophosome. The bacteria oxidize sulfur from the worms environment to produce the nourishment the animal needs. Using hydrogen sulfide as the energy source, the reaction for chemosynthesis is: 12 H2S 6 CO2 → C6H12O6 6 H2O 12 S This is much like the reaction to produce carbohydrate via photosynthesis, except photosynthesis releases oxygen gas, while chemosynthesis yields solid sulfur. The yellow sulfur granules are visible in the cytoplasm of bacteria that perform the reaction. Another example of chemosynthesis was discovered in 2013 when bacteria were found living in basalt below the sediment of the ocean floor. These bacteria were not associated with a hydrothermal vent. It has been suggested the bacteria use hydrogen from the reduction of minerals in seawater bathing the rock. The bacteria could react hydrogen and carbon dioxide to produce methane. Chemosynthesis in Molecular Nanotechnology While the term chemosynthesis is most often applied to biological systems, it can be used more generally to describe any form of chemical synthesis brought about by random thermal motion of reactants. In contrast, mechanical manipulation of molecules to control their reaction is called mechanosynthesis. How Procrastinating Writers Should Work Just today I finally checked an item off of my highly vague To Do Eventually list, an item I had put on the list more than a year ago. I tried to make myself feel less guilty by titling the list with eventually but  really.  A year. It took me a year to do it. That stupid task had been nibbling at my conscience, as writer  James Surowiecki aptly put it. Lets get one thing clear: procrastination isnt bad. We assume it is, because it puts us in a high-stress rushed state when its finally time to pay the piper for deadline projects;  but procrastination is merely another working style, and not merely an example of a bad working style. Lets get one thing clear: procrastination isnt bad. Guilt-ridden procrastinators (of which I am one, and terribly) spend much of the time they arent doing their work reading about how to stop procrastinating. They look for methods to be more productive, to be the early bird that gets things done in a flash and cant stop checking things off of the list. They buy books, organizers, and apps. And still procrastinate. And are frustrated. There are two ways to approach your procrastination: fight it or work with it. Working with your procrastination. In a broad sense, procrastination has a funny way of getting our priorities straight. Look at your to-do lists. How many of them did you start with specific tasks that had specific dates until, in your eagerness, things got a little out of hand and soon you had a massive list of things you ought to do and should do and might do? Procrastination has a way of helping us not get caught up in what we subconsciously determine is unimportant. Procrastination allows perfectionists to get things done by forcing them to do adequate work under self-induced deadline pressure when they otherwise would be unable to do any work. Procrastination can keep us from fixating on things that are unimportant.In that sense, it is good procrastination. It keeps us from sweating the small stuff. The unimportant things eventually disappear if you never do them. As long as your procrastination has you meeting deadlines and getting the big stuff done, its helping you out. My solution is to use triage rubrics in different areas of life, and to hold my to-do list up against the rubric. For example, in my personal life, family always trumps work. In my work life, client deadlines before tweaking my own website. This is an over-simplification, but I know that if I have these ingrained, I dont fret about the things I procrastinated on that fall into the category of unimportant. Create your own rubric. Start at the top with the thing that will get you into serious trouble if it isnt finished and go back from there. Train your mind to understand what is important. You should work with your procrastination instead of fighting it if: You make to-do lists that are massive and full of unimportant tasks. You have not missed deadlines. You are getting important things done. Fighting your procrastination. Of course, there are times when procrastination is a bad idea, such as putting off paying your taxes and ending up with a penalty because of it. If youre missing deadlines or feeling high levels of stress because everything feels last minute, thats bad procrastination. These undone things dont disappear, they get worse. For writers, bad procrastination means you dont have time to proof and edit properly. You have to constantly deal with reminders and demands of clients wonder why your work isnt submitted. Your work and reputation suffers. This is where the  Zeigarnik effect  comes into play. Bluma Zeigarnik was a psychologist from Lithuania who noticed that restaurant servers could remember large amounts of information (without writing it down) for a limited period of time. Once the food was delivered to the table, servers forgot it all entirely. Through studies Zeigarnik learned that we remember interrupted tasks better than non-interrupted tasks. For procrastinators, this is good news. Once we start a task, we gain focus. This is helped if we arent interrupted. The Zeigarnik effect reveals that starting anywhere on the task is the path to getting it done, even if its with an easy part.  When we allow distractions and interruptions, that unfinished saved-for-later task nags at us relentlessly until we finish it. Start small.If you are procrastinating on writing a post, start with a small thing first. Begin collecting reference links and doing research. Brainstorm ideas. Try free writing techniques just to get text on the screen. Go where you wont be interrupted. Think of the restaurant server. You cant bring the food to the table until youve taken the order and delivered the beverages. Start easy.If getting started is proving impossible, try the  Pomodoro technique. Break up your work into 25-minute timed segments with a five-minute break in between. After four work periods, take a 20 minute break. Once you get in that writing zone skip the breaks and go with it. This way, you know at the start you get breaks and you can ease into the project. How List-Oriented  Writers Should Work A great chef or line cook knows what  mise-en-place  is. It means that before you even begin cooking, you have your station ordered with everything where it belongs. This way, when the rush begins, you are not scrambling for tools and ingredients. As a pastry chef, before I started actually making a recipe, I always got the tools and ingredients together first. It kept me from wasting precious time in a busy day, and it also kept me from starting something and discovering we were out of eggs halfway through. Writer Ron Friedman describes the concept aptly: the single most important ingredient of any dish is planning. This concept appeals to writers who like to make lists. To-do lists, idea lists, project lists, supply lists, editorial calendar liststhese are the people who want things in order. These are the people who are constantly planning. Friedman applies mise-en-place to any kind of work, asking a great question: what is the first thing you do when you start work? Do you check your email? Your Twitter feed? Your analytics from yesterdays blog post? Your voice mail?  These are activities that, according to Friedman, put you into a reactive mode. They make us lose our focus, and let other peoples priorities take center stage, Friedman said. They are the equivalent of entering a kitchen and looking for a spill to clean or a pot to scrub. Begin your day with a brief planning session. An intellectual mise-en-place. Ron Friedman Youre not in a good place to write after youve started with these activities, but instead youre in that no-win zone of reacting and catching up. The truth is, the emails never stop rolling in, the tweets dont stop chirping, and yesterdays analytics can be analyzed later. And people who make lists are prone to starting the day reactively. Or, I should say, people who make lists without hierarchy are prone to starting the day reactively.  Your lists of things to do will hamper you if you do not structure them with the idea of mise-en-place. How do you make a list that works? 1. Small tasks, action words. David Allen, of the well-known Get Things Done system, suggests that you break down  your tasks on your to-do lists, and start the smaller components with action verbs. Instead of: Blog post due try Write 15 headlines. Find 5 outside resources. Write introduction. Outline blog structure. Write first draft. Blog post due isnt mise-en-place. Its we have knives somewhere in the kitchen. Its vague. It tells you a deadline, and not what to do. 2. Prioritize your tasks. Your willpower is at its greatest in the morning. This means that you should  prioritize the things you have to do (not react to) by scheduling them first, in the morning. Leave the easier, less mentally challenging tasks for later. You can answer emails in the afternoon, when your mind is slowing down a bit, but youd better use that morning mental acuity for writing your content.

As the patient has iron deficiency anaemia a blood transfusion is necessary to increase  haemoglobin levels within the blood as this helps to transport oxygen to cells and tissues. She also  has a history of PR bleeding. Therefore this blood transfusion is helping to replace volume lost, to  increase circulating blood volume and to improve the oxygen carrying capacity (Hamlin, Richardson-Tench, Davies 2009 pp 155,156) c. It is important to follow the Pico prep instructions as faecal matter can obscure the viewing of the  the colon. Pico prep aims to thoroughly cleanse the colon of any matter or gas to ensure that the  visual field is clear ( Corbett Banks 2011 pp. 675,676). d. Pico prep is an osmotic laxative, its action decreases the fluid absorption within the bowel which  then results in the onset of diarrhoea within 1-4 hours. Lowering the bed, having her four wheel walker in reach and the application of  hip protectors may aid in reducing the risk of her having a fall and in the chances of her having a fall  the hip protectors may aid in protecting that area.( Crisp, Taylor, Douglas, Rebeiro 2013. p. 454). Providing a bedside commode may also reduce the chances of falls as it is located closer to her than  the toilet may be. As she is an older patient the skin around the area may become excoriated and skin  breakdown may occur due to the acidity of the diarrhoea and the area frequently being wet. Barrier  creams should be applied to at risk areas for protection. Diarrhoea can also quickly cause dehydration  and electrolyte imbalances in the elderly, this may also have an impact on this patients fluid and  electrolyte levels (Williams Hopper 2011. pp. 275, 747). Question 2. a) This patient is displaying possible signs and symptoms of a suspected urinary tract infection such  as incontinence, a burning sensation when she voids, fever, confusion and blood stains on her pad. A urinalysis should be performed to support a diagnosis of a urinary tract infection ( Williams   Hopper 2011 p. 838). As she is incontinent of both urine and faeces a thorough skin assessment  should be performed to identify the areas at risk and to identify any change in skin integrity. Skin  turgor should also be assessed as this can indicate a sign of dehydration (Crisp et. al. 2013 p. 592). A  fluid balance chart should be maintained to assess if the patient is in a positive or negative fluid  balance and the weight of the patient should also be assessed as noticeable weight changes can  indicate hypovolaemia (Crisp et.al 2013 p.1214, Scott 2010 p. 62). Auscultation of the chest could  prove useful in determining the reason of the increased respiratory rate and low oxygen saturation  levels ( Lewis Foley 2011 p. 356). A falls risk assessment should also be performed as the elderly  patient has a few risk factors for falls such as confusion, reduced mobility an d is incontinent of urine  and faeces. This can help to implement interventions to reduce the risk of a fall ( Crisp et.al p. 454). As this patient is at risk of both hypovolaemia and hypokalaemia the doctor should be notified to  thoroughly assess the patient and implement therapy for a suspected urinary tract infection. b) Cranberry juice can be effective in helping to reduce pain when urinating and also prevents the  bacteria adhering to the wall of the bladder, this method can be helpful in reducing the pain of a  urinary tract infection however the patient is undergoing a procedure the next day, therefore this  intervention should be implemented with the approval of a medical officer. A heat pack could be  placed on her abdomen to relive any pain and discomfort along with the administration of an  antipyretic to reduce her fever and pain (Williams Hopper 2011 p. 840). As the patient is having  difficulty breathing she should be placed in a suitable position to help with proper lung expansion such  as the high fowlers position along with the administration of oxygen to increase oxygen levels within  the blood. (Williams Hopper 2011 p. 604). The patient’s vital signs should be continuously  assessed to monitor any improvements or deterioration especially her blood pressur e and heart rate  as any further abnormalities such as arrhythmias and a further decline in blood pressure could  indicate hypovolaemia and hypokalaemia. Continuous assessment of her neurological state should  also be implemented to monitor any changes (Scott 2010 p. 64). c. Hypokalaemia occurs due to an excessive loss of potassium from the body or from an inadequate  intake of potassium. The body is unable to conserve potassium and relies on an adequate intake of  potassium to maintain a balance within the body. An excessive loss of potassium can be due to  diuretic therapy – especially potassium wasting diuretics, corticosteroids, vomiting and diarrhoea. Signs and symptoms include an irregular weak pulse, hypotension, muscle cramps, muscle weakness  and shallow respirations. (Williams Hopper 2011 p. 79, Scott 2010 p. 98). Medical management is aimed at restoring potassium levels either by increasing the intake of  potassium in the diet or oral potassium supplements. Intravenous replacement therapy is also  implemented in those with severe hypokalaemia to rapidly increase potassium levels. Diuretics may  be changed to a potassium sparing diuretic to prevent the loss of potassium from the body. (Scott  2010 pp. 100,101). Nursing management includes monitoring fluid input and output, monitoring the heart rate and rhythm  of those receiving IV replacement therapy, maintaining and ensuring the correct administration of the  therapy and continuous monitoring of the patient’s condition throughout. ( Scott 2010 p.102). Hypovolaemia occurs due to the loss of fluid from the body and extracellular spaces; this can be due  to excessive bleeding, excessive sweating, burns, diuretic therapy, diarrhoea, renal impairment and  vomiting. The loss of fluid then results in a decreased blood volume. (Williams Hopper 2011 p.71,  Scott 2010 pp. 60, 61). Signs and symptoms include thirst, nausea, hypotension, restlessness,  confusion, dizziness, cool pale skin, tachycardia, increased body temperature, weight loss and a  decline in cognitive status. (Williams Hopper 2011 p 72, Scott 2010 p. 62). Medical management includes finding and stopping the source of the fluid loss, the replacement of  lost fluid with an intravenous infusion with the same osmolality of blood to increase the body’s blood  volume. ( Scott 2010 p.63). Nursing management includes the administration and maintenance of intravenous fluid replacement,  monitoring the daily weight of the patient, monitoring fluid input and fluid output, encouraging the  intake of fluids to aid in restoring fluid balance and providing mouth care to maintain the integrity of  the oral mucous membranes. (Crisp et.al. p. 73). Question 3 a) Midazolam is used in this procedure as it is a sedative, hypnotic agent and muscle relaxant. This  aims to reduce the amount of movement throughout the procedure and assists in keeping the patient  in a sedative state and impairs memory function ( Tiziani 2013 p. 967). Fentanyl would be used to  reduce pain during the procedure and also aids in the maintenance of the anaesthesia ( Tiziani 2013  p 928) Diprivan is used to induce sedation and also increases the effects of the hypnotic agent and  analgesia ( Tiziani 2013 p 793.) b) Midazolam acts by binding with a benzodiazepine receptor in the central nervous system which  inhibits neurotransmitters in the brain resulting in a calming sedative affect ( DrugBank, Midazolam  DB00683 2013). Midazolam given intravenously takes affect within 1.5 2.5 minutes. Adverse effects  include respiratory depression, memory impairment, anxiety, muscle weakness, drowsiness,  hypotension, dizziness, fatigue and decreased alertness. (Tiziani 2013 pp 964, 967) Fentanyl acts on receptors within the brain, spinal cord and muscles and bind with opioid receptors  producing an analgesic affect. Administered intravenously fentanyl takes affect almost immediately.   Side effects include respiratory depression, apnoea, dyspnoea, vomiting, nausea, increased intra  cranial pressure, bradycardia, sedation, confusion, constipation, hypotension and muscle rigidity.  (Tiziani 2013 p. 923) Diprivan suppresses the central nervous system and produces a loss of consciousness. Adminstered  intravenously diprivan takes affect within 30 seconds of administration. Side effects include  respiratory depression, tachycardia, hypotension, shivering and involuntary muscle movements (Tiziani 2013 p 793) Nursing care includes continuous monitoring of respiratory rate, heart rate and vital signs during  administration of these agents and throughout the procedure, ensuring that the dose is titrated to  produce the right affect, a sedation scale should be performed when the patient is conscious,  ensuring that the patient is aware that midazolam can cause muscle weakness so care should be  taken when mobilising. Central Nervous System toxicity may occur when all three medications are  given together therefore continuous monitoring is extremely important as the effects on the central  nervous system are increased ( Tiziani 2013 p 964,968). c) As this patient has renal failure the kidneys ability to filter and excrete waste is decreased, this may result in an accumulation of the medications and could possibly result in drug toxicity – especially  opiate medications (Tiziani 2013 p.925). This patient is elderly and may have increased sedation and  confusion after the procedure due to her age and renal function and is at a high risk of falls especially  as midazolam causes muscle weakness. Midazolam administered to an elderly patient can cause  delirium, therefore this patient is at an increased risk of being affected by this ( Tiziani 2013 p.964). Prufrock(the narrator) believes that age is a burden and is deeply troubled by it.. His love of some women cannot be because he feels the prime of his life is over. His preoccupation with the passing of time characterizes the fear of aging he has. The poemdeals with the aging and fears associated with it of the narrator. Prufrock is not confident with himself mentally or his appearance. He is terrified of what will occur when people see his balding head or his slim and aging body. He believes everyone will think he is old and useless. They will talk about him behind his back. (They will say"How is hair is growing thin!") My morning coat, my collar mounting firmly to the chin, My necktie rich and modest, but asserted by a simple pin-- [They will say: "But how his arms and legs are thin!"] This insecurity is definitely a hindrance for him. It holds him back from doing the things he wishes to do. This is the sort of characteristic that makes Alfred into a tragic, doomed character. He will not find happiness until he finds self-assurance within himself. The repetition of words like vision and revision, show his feelings of inadequacy in communicating with the people around him. J. Alfred Prufrock's self esteem affects his love life greatly. The woman he is in love with is younger than he is and this distresses him. He does not believe that some younger women could possibly accept him or find him attractive. Expressing any kind of affection to her is awkward and difficult. Prufrock knows what he must say but cannot bring himself to say it. "Should I, after tea and cakes and ices, Have the strength to force the moment to it's crisis?"(79-80) His apprehensiveness in his love life, is very troublesome for him indeed. He wishes greatly to express his affection but it becomes suppressed within him. He compares himself to Lazarus who was an aged man restored to life by Jesus. Civil Disobedience: Henry David Thoreau and Rev. Dr. Martin Luther King Jr

â€Å"Disobedience to be civil has to be open and nonviolent. † – Mahatma Gandhi Throughout history philosophers have played a key role in our society. Both Henry David Thoreau and Dr. Martin Luther King Jr. brought forth their own ways of civil disobedience, in their belief that it was imperative to disobey unjust laws. Their thoughts manifested from ideas, to theories, and eventually lead to our society today. Civil disobedience in a pragmatic way is the act of a non-violent movement in order to enforce the change of certain laws to ensure equality for all. Dr. King explained in his quote â€Å"One who breaks an unjust law must do so openly, lovingly, willingly to accept the punishment† (220). Nevertheless, on opposite ends of the spectrum, Thoreau implied an aggressive stance motivated by his own personal hate for the government but yet King used religion, supported by his charismatic ways of being gentle and apologetic. While King and Thoreau both believed in the use of civil disobedience to create change, they went about using civil disobedience in staggeringly different fashion. As stated by Dr. King in his letter from Birmingham Jail, â€Å"Injustice anywhere is a threat to justice everywhere† (214). Regarding this issue, King believed that all American communities are connected and that injustice in one community will affect other communities. Perhaps, one could deem injustice as a disease such as cancer that forms in one area then quickly spreading and eventually discombobulating the entire social infrastructure. Dr . King reshaped America’s social issues through a non-violent approach in distinction to boycotting buses in Montgomery to marching through Selma, King responded to unjust laws with civil disobedience and direct action. Dr. King’s stance on prejudice laws came from morality. Primarily using morality as a backbone in his argument, we would agree that it is wrong to foster laws that affect a certain race or group of people. Moreover, our laws are a reflection of our morals and it sets forth what we know is right and what we know is wrong. Early philosophers often struggled and faced opposition with either the government or social groups. Opposition faced consequences such as confinement, torture, or worse, death, whereas the idea of brutal punishment inflicted fear on the next individual. In his â€Å"Letter from Birmingham†, King compared his calling to Birmingham to the Apostle Paul in the Bible, â€Å"[and how he] carried the gospel of the lord to the far corners of the Greco-Roman world† (214). King expressed a legitimate concern over the anxiety to break laws; elaborating the fact that there are two laws; just laws and unjust laws. King stated, â€Å"In no sense do I advocate evading or defying the law† (220). Rather more, King agreed that just laws should be followed; however unjust laws are to be met with civil disobedience. What makes a law unjust one might ask? From the terms of St. Thomas Aquinas, King explained that â€Å"any law that degrades human personality is unjust†. (219) Segregation gives the segregator a false sense of superiority and distorts the soul and damages the personality. Back in Dr. King’s time, a series of laws were passed that were the ethos of â€Å"separate but equal†. King rallied in oppositi on of these laws as still prejudice and unjust, in fact these laws were against morals. Under this doctrine, services, facilities and public accommodations were allowed to be separated by race, on the condition that the quality of each group's public facilities was to remain equal. Signage using the phrases â€Å"No Negros allowed† and â€Å"whites only† distorted our views on race relations. However, King believed this in fact is not equality and it is against our morals. As a result of Henry David Thoreau using civil disobedience and direct action, Dr. King was motivated by his techniques which lead to a series of events that would lead to the Civil Rights Movement. â€Å"All men recognize the right of revolution; that is, the right to refuse allegiance to, and to resist the government when its tyranny or its inefficiency are great and unendurable† (180). As Thoreau explained in his excerpt from â€Å"Civil Disobedience†, Thoreau used the revolution of ’75 as an example of bad government. Thoreau elucidated how the government taxed certain foreign commodities that were brought to its ports. He then began to correlate bad government to a machine and stated how all machines have their friction, however, when friction takes over a machine, â€Å"and oppression and robbery are organized, I say let us not have such a machine any longer† (180). Thoreau elaborated on this idea that the government is a machine and when evil takes over, let us no longer have such a government. He believed not that a government should exist â€Å"but at once a better government† (178), Thoreau argued that power should not be left to the majority, but the â€Å"conscience†, in fact he questioned the reader rhetorically asking â€Å"Must the citizen ever for a moment, or in the least degree, resign his conscience to the legislator? †(178) Thoreau feels that the â€Å"conscience† plays a personal role. Thoreau questions democracy, and thereupon he advises us to question why we should capitulate to the government if we do not agree with a law? Why would we possess brains and have a conscience of our own if we are not allowed to think for ourselves and do what we want? Thoreau feels we ought to be real for ourselves, not the government. Furthermore, he articulated the idea that should we surrender our thoughts, or conscience to the government, or should we pursue a justifiable explanation of the dilemmas that surround us? What is right as opposed to what is wrong is what leads to civil disobedience. Thoreau believed that the idea of paying taxes to support the Mexican-American was an unjust cause, whereas; King strongly disagreed with laws that were prejudice. In Thoreau’s reading from his article â€Å"Civil Disobedience†, he argues â€Å"that government is best which governs not at all† (177), which ultimately leads the people to discipline themselves. On the other side King explained how â€Å"nonviolent direct action seeks to create such a crisis and foster such a tension that a [community that has refused, is forced] to confront the issue† (216). By cause of King being after Thoreau’s era, King used Thoreau’s â€Å"Civil Disobedience† and direct action to spark a change in society. While both Thoreau and King argued with morality in mind, they both believed injustice exist. Thoreau thinks of injustice as friction or tension that can wear the machine down. King believes that injustice just exists and tension must be created with direct action to negotiate with the machine. I accredit Dr. King in presenting the best argument due to the audience he reached out to which of course was the populace and his motives that captivated his courageous and selfless acts. Furthermore, Dr. King was concerned about injustice towards people based on their race, religion, or sex; whereas Thoreau was motivated by his personal hatred for the government. Regardless of how either King or Thoreau used civil disobedience, their contributions led to an admiration for their works and casted a light on unjust laws.
